HR documentation process for General physician clinics: records that stay in the clinic
A small clinic keeps a lot on paper: staff files, certificates, letters and registers. When these are scattered or one person holds the only copy, the clinic loses time, and sometimes loses an argument.

Share your requirementsWhy HR documentation process matters in general physician clinics
Clinic owners usually keep staff papers in a drawer or on the doctor's phone. An appointment letter may be a message. Registration and training certificates are photographed and lost when the phone changes. When a dispute arises or a licence renewal needs a document, the search begins. Nobody is sure which version is current.
A documentation process lists the papers each role needs, keeps them in one place and sets who can see them. It defines a simple joining pack, a file for each person and a calendar of renewals. It also separates staff records from patient records, which have their own rules, and reminds everyone that the second kind must stay in the clinic.
How to set it up
1. List the papers per role
For nurse, receptionist, dispenser and locum, list what you need at joining and what must be renewed. Mark expiry dates.
2. Make a joining pack
Appointment letter, role page, pay terms, leave rule and records handling note. Both parties sign and keep a copy.
3. Keep one file per person
A folder, paper or digital, held by one named person. Set who may open it and where it is stored.
4. Set a renewal calendar
Mark registration and certificate renewals for staff and locums, and check it monthly. See employee information management.
5. Separate patient records
Write a short note on what patient papers are, where they stay and who may take them out. Confirm the rules with a qualified professional.
Check these first
Before changing anything, find out what is true today. These take an afternoon and use your own records.
- Check whether every staff member has a signed appointment letter.
- Look for a certificate that expired without anyone noticing.
- Ask who holds the staff files and who could open them.
- See if you can find a locum's papers in under five minutes.

Questions owners ask
Keep what you need in a form you can find and protect. A phone alone is fragile. Use a shared folder or a locked cabinet and let more than one person know where things are.
Appointment letters, pay terms, registrations and certificates, and any warning or exit paperwork. These are what you look for when something goes wrong.
A single named person, such as the clinic manager, with a deputy. Limited access protects privacy and avoids missing pages.
