Professionalism checklist

A checklist of what supervisors can see and note, not guesses about attitude. Tick what you observed over the last [4] weeks, add one example, and agree one action. Use it for coaching, not as a score on its own.

Note on use

This checklist supports coaching. If it feeds into pay, promotion or discipline, check the process against your policies, contracts and applicable rules with a qualified adviser.

A starting point, not legal advice. Check the final wording against your contracts and the rules of your state, and have a qualified professional review it before you issue it.

How to use this template

  1. Brief supervisors on what each item looks like in your workplace before first use.
  2. Observe over several weeks, not one bad day.
  3. Mark only what you saw. Write an example for every N.
  4. Hold a private conversation. Start with strengths, then agree one or two behaviours to improve.
  5. Follow up at the date set and record the change.
  6. Keep the form in the employee file, and apply it the same way to every team member.

Common mistakes

  • Marking attitude or personality instead of things you saw.
  • Judging on one incident rather than a period.
  • Using the checklist as a surprise before a pay decision.
  • Applying a stricter standard to some people than others.
  • Ticking boxes without an example or a follow-up action.

Adapt it to your business

  • Factory or plant: add safety gear use, shift handover and line discipline.
  • Shop or retail: add customer greeting, cash handling and store presentation.
  • Clinic or hospital: add patient respect, hygiene and privacy.
  • Office or services firm: add meeting etiquette, client communication and response times.

Questions about this template

It is consistent, observable behaviour: being on time, communicating clearly, owning work, respecting others and keeping information safe. It is judged by what people do, not by how they dress or speak.
