Clinic papers often live in several places: the owner's email, a front desk drawer and a therapist's phone. When a patient complains or a therapist leaves, nobody can find the offer terms or the course certificates. A new manager is left guessing what was promised.
A documentation process fixes this with a short list of documents every person must have, a standard format for each and one named person who looks after them. It does not need software. A shelf, a folder structure and a quarterly check are enough for a clinic of this size.
How to set it up
1. List the documents each person needs
Offer letter, role description, joining form, identity papers, degree, registration, course certificates and notice terms. Make it a one-page checklist.
2. Write plain role descriptions
A page per role stating duties, reporting line, hours, home visit expectations and the support provided. This also helps role design later.
3. Use standard offer and exit letters
One template for therapists, one for assistants and one for the desk, reviewed by a professional you trust.
4. Name a keeper and a place
One person, usually the clinic manager, owns the files, which are kept locked and listed. Say who may see what.
5. Check renewals every quarter
Go through each file for expiry dates and missing papers. Write what needs action and who will do it.
Check these first
Before changing anything, find out what is true today. These take an afternoon and use your own records.
- Pick two employee files at random and see how many checklist items are present.
- Ask where a therapist's registration paper is kept right now.
- Check that every employee has a signed role description.
- See who can open the files and whether that is written down.

Questions owners ask
Yes. They settle questions about duties, hours and home visits before they become disputes. One page per role is enough, and it helps when you hire or review pay.
The period depends on the type of record and the rules that apply to your clinic. Ask a qualified professional and write the answer into your checklist so that nobody discards papers early.
Either can work if it is orderly, protected and backed up. The main point is that the files are complete and that somebody is responsible for them.
