Attendance, shift and leave process for Paediatric Clinics: evening peaks and weekend cover
A children's clinic fills up after school and on weekends, while the morning stays calm. Rosters and leave rules need to follow those hours, or the same few people carry every busy session.

Share your requirementsWhy attendance, shift and leave process matters in paediatric clinics
Many paediatric clinics build the roster from habit. The same receptionist closes every evening, the same nurse takes Saturdays and leave is granted on a first-come basis. Flu season arrives and three people ask for time off in the same week. The clinic stays open, but the people on shift are tired and parents see it.
A better process starts from arrivals. It shows when the desk and the injection room are busy, sets the number of people for each period and rotates evenings and weekends fairly. Leave is planned against a calendar of known pressure points such as vaccination camps, school health visits and festival seasons.
How to set it up
1. Map arrivals by hour
Take four weeks of token times and plot arrivals for each day. Mark the evening peak, the Saturday morning rush and the slow patch after lunch.
2. Set minimum cover per period
Decide how many desk staff, nurses and helpers must be present at each period. Write it as a simple table the manager can follow.
3. Rotate evenings and weekends
Share late closings and Saturdays across staff in turn, with a note of who has worked what. Fairness here prevents quiet resentment.
4. Keep a pressure calendar for leave
List camps, flu months, school terms and festivals. Ask staff to give early notice for leave in those periods and agree swaps in writing. See the attendance, shift and leave process.
5. Record attendance daily and review monthly
One simple record for every person, checked by the manager each month for patterns such as repeated late arrivals or unplanned absence on camp days.
Check these first
Before changing anything, find out what is true today. These take an afternoon and use your own records.
- Compare the number of staff present at 6 pm with the number of tokens issued around that time.
- Count how many Saturdays each person has worked in the last quarter.
- See whether any leave request in flu season was approved without checking cover.
- Check that camp days appear on the roster at least a week ahead.

Questions owners ask
Keep a simple chart of who closed and who worked weekends. Rotate in order and allow swaps with the manager's agreement. People accept long evenings when they see everyone takes a turn.
A clinic can ask for early notice and limit overlapping leave, if the policy is written and applied evenly. Check the rules for your state with a qualified professional, and explain the reason to staff.
Keep a named backup for each camp. If none exists, move the camp or reduce the number of children rather than overload the rest. Record what happened and plan cover next time.
