Attendance, shift and leave process for Home nursing agencies: rosters that survive a sick call at midnight
A home nursing roster is rebuilt daily as cases begin, extend and stop. Without a clear process, the coordinator's phone becomes the only roster and one sick call can empty a night duty.

Many agencies run rosters from one coordinator's notebook or chat group. It works while she is on duty and fails the day she is ill. Nurses hear of a shift late, ask for swaps by message, and the family is told only when someone fails to turn up. Leave is granted by whoever answers first.
A workable process keeps one live roster per case, a standby list, and a daily morning check of the next two days. It records arrival and departure at the home. It sets fair rules for night duty, festival duty and leave so that the same few people are not always asked.
How to set it up
1. Keep one roster for each running case
List the patient's address, hours, regular nurse, backup nurse and any special routine. Update it whenever the family changes a duty and tell the nurse the same day.
2. Run a two day look ahead each morning
The coordinator checks the next two days' duties, confirms each nurse by call and fills gaps from the standby list before they become emergencies.
3. Record arrival and departure
The nurse checks in and out by call or message at the home. A weekly sheet signed by the family is matched against it, so hours are agreed at the time.
4. Set fair rules for night and holiday duty
Rotate the hard duties, say how swaps are requested and who approves them, and write the rules where every nurse can see them.
5. Plan leave around known case dates
Ask for leave early, check it against the running cases and arrange cover before approving. The HR process consulting work can help design the form.
Check these first
Before changing anything, find out what is true today. These take an afternoon and use your own records.
- Count the duties in the last month that started late or were covered at short notice.
- Ask a coordinator what happens to the roster if she is absent for a week.
- Compare one nurse's recorded hours with the family's sheet for the same week.
- See whether the same three people took most night and festival duties.

Questions owners ask
Have a standby list and a rule that sick calls are made as early as possible. The coordinator calls the standby, briefs her on the case and informs the family. Record what happened so patterns show.
Swaps can work if they go through the coordinator, who checks the replacement knows the case. Unrecorded swaps are the cause of many disputes, so the rule must be easy enough that people follow it.
Agree a simple rule at the start for changes, and have the nurse inform the coordinator the same day. The roster is updated, and the nurse's hours are recorded rather than argued about later.
