HR documentation process for Ultrasound Centres: records that stand up when someone asks
An ultrasound centre handles papers of every kind: staff files, visiting doctor terms, registers and consent forms. Gaps are found only when someone asks, so a steady routine matters.

Documents pile up in small centres. A staff certificate is in one drawer, a doctor's session terms are in an email, and the register is signed in a hurry at the end of the day. When a question arrives, from a patient, a doctor or an inspector, the owner spends hours searching, and some papers cannot be found at all.
A light routine fixes most of this. Each person has one file with the same contents. Each visiting doctor has written terms. Daily registers are checked and signed by the supervisor. None of this needs software, only a list of what must be kept, who keeps it and when it is checked. Confirm the retention rules with a qualified professional.
How to set it up
1. List what every staff file holds
Appointment letter, identity and address papers, qualification and registration copies, role page and emergency contact. Write the same list for every person.
2. Write terms for visiting doctors
Record session days, fee basis, reporting expectations, conduct and notice, signed by both sides. Keep the signed copy in a named file.
3. Set a daily register check
The supervisor compares the day's list with registers and report copies before closing, signs, and notes gaps. This takes ten minutes if done daily.
4. Run a monthly file review
Pick five staff files and check against the list. Note missing items and give a date to fix them. Rotate so every file is checked within a year.
5. Fix where papers are kept and who can see them
One locked cabinet or access-controlled folder, a named keeper and a record of who took what. Patient records and staff records are kept apart.
Check these first
Before changing anything, find out what is true today. These take an afternoon and use your own records.
- Pick three staff files and compare them against your list.
- Find the written terms for each visiting doctor.
- Check yesterday's register against the appointments list.
- Ask who can open the staff cabinet and whether access is logged.

Questions owners ask
Retention periods depend on the document and the rules that apply to your centre, so ask a qualified professional. Meanwhile, keep records safely and do not destroy anything until you are told.
It can be, if it is complete, locked and checked. Digital copies add safety against loss, but the main thing is a routine that someone follows each month.
A named person, usually the supervisor or centre manager, with the owner checking monthly. Without a name, the routine fades within a few weeks.
